If you wish to add, delete or amend commercial equipment within your policy, please complete all the information below for each piece of equipment and submit it to us at MHK Insurance. We will then advise you once your information has been updated at the email address you provide below. Please be advised that your change is not in effect until you receive this confirmation from us.
Based on the type of equipment being added, this will determine the level of coverage. Please refer to your schedule of coverage for details.
